Health plan and individual needs

Nathan is 26 years old and is addicted to cannabis. He doesn’t work and is on ESA (Employment Support Allowance) and DLA (Disability Living Allowance). He is housed by the council and lives with his girlfriend who is also his career. He very rarely goes out unless he has to but it will only be for himself or if he has an appointment somewhere, or to get more cannabis. He suffers from Anxiety, which causes him to feel worried, nervousness, or unease, typically about an imminent event or something with an uncertain outcome.

He also suffers with Impulsive Anger Disorder which is a behavioral disorder characterized by extreme expressions of anger, often to the point of uncontrollable rage another illness his suffers from is Psychosis which is a severe mental disorder in which thought and emotions are so impaired that contact is lost with external reality. He’s supposed to take medication but doesn’t take them regularly. Because Nathan smokes cannabis it cause’s all his illness to be worse. Nathan is on more then one place on Maslow Hierarchy of Needs.

Nathan is on the Physiological need as he has food, water, sex, sleep; He also just comes into the Safety needs on Maslow’s chart as he does have property. Nathan also comes under the Love and belonging need as he has family, sexual intimacy and a small amount of friends. Start taking medication called Seroquel regularly to help with your psychosis. 50mg twice a day. Depending how things are going see if your doctor can lower your does to 25mg or higher your does to 100mg if you feel they are not taking affect.

We’ll recap you’re progress and agree to an mg to keep you on all the while its helping. And make appointments with the Community psychiatric nurse. Psychiatric nurse, family and friends. Anxiety. Drug therapy. Start taking medication called Pregabalin regularly which will help you’re Anxiety. 50mg 3 times a day. Depending how things are going see if your doctor can lower your does to 25mg or higher your dose to 100mg if you feel they are not taking affect. We’ll recap you’re progress and agree whether you need to keep taking this medication or slowly take you off.

Psychiatric Nurse, family and friends. Impulsive anger disorder/ Depression. Start taking medication called Citalopram regularly to help with your depression and impulsive anger disorder. 50mg once a day in the morning or night. Arrange appointment to see anger management. Depending how things are going see if your doctor can lower your does to 25mg or higher you’re does to 75mg if you feel they are not taking affect. We’ll recap your progress and agree whether we are able to lower your dose or slowly take you off this medication. Psychiatric Nurse, family and friends.

My health plan meets the wellbeing and needs of my individual by helping him understand the key factors which are triggering his illnesses and addiction that seems to make him have anger bursts, mild psychosis episodes, stages of feeling alone and also occasional moments of being paranoid. The main key factor of his problems is cannabis abuse. My individual and I have agreed that smoking cannabis doesn’t help with his psychosis, impulsive anger disorder, depression or anxiety as it causes him to forget and also makes him feel he doesn’t need to take his medication.

Making him this health action plan will help him understand what he needs to do to make his life better, which will give him a healthier and more stable environment to live in. This action plan is also giving him the understanding that taking his medication regularly will give him the control over his psychosis and other illnesses and to stop them controlling him. I’ve also advised that he consults with his doctor on regular bases about the dosage of his medication and how often he should take them.

This will also give him a sense of control which is a very key point with his whole situation. Also making a plan of cutting down smoking and looking into ways that can replace the urge of want to smoke. I’ve also advised him to find a hobby as well to take his mind off smoking cigarettes and his other drug taking. We will be recapping how his hobby and progress is going and if it hasn’t helped maybe look at joining a club or a course of some sort.

With him doing something with his time we have both agreed that is will be a major help with his anxiety and depression. Once we’ve established the best course of action we will then eventually try to quit smoking entirely which will give him more energy, money and a much better health. My health plan also recognises my individual’s circumstances and preferences by realising what his circumstances are and how his health is affected and what needs to be done to improve how he is living.
