There have been complaints about physicians on the issue of physical query form. They do not seem to understand what it means and its use in health care delivery. Therefore, I write to you to give an outline of a 30-minute speech where I will address this issue in detail. The 4 quadrants I will like to cover is coded as Definition/Introduction of term: What is Physician query form? Here I will give account of the meaning of the form, define closely related terms.
Historical Perspective: I will give a historical account of the form, who first mentioned it and where? How was it introduced in medical practice, when and how? How has it changed over the years in form and design? Derivative of the Form: Of what use is the physician query form? What is the cost of using the form? How does one use it? Direct relationships: What relationship exists between this form and medical practice?
How does this affect physician-patient relationship, and relationship between physician and other health care service providers? These are the four areas I will cover in the speech, will also give room for question and answers. At the end of the session, we shall also develop FAQs [Frequently Asked Questions] on coding/physician query form which will made available for physicians and coders.
